Is there any scary and ugly looking female fictional characters, in comics, or other graphic novels, that also have a highly realistic non-fantasy appearance, if yes like what, and how can we make such a character, when it it comes to her different facial and physical features?

Theres Sakura Ogami from Danganronpa. Although to be fair her entire shtick is supposed to be parodying the "school girl fighter" by making her ridiculously buff (also just realised you asked for comic book characters and she is a video game character.... I think she was in a manga?)

There's Despair from Sandman although she is an overworldly character albeit with a realistic appearance

Dorothy Spinner from Doom Patrol, whose entire story arc was coming to terms with her appearance

im aware the proposals I have are all unattractive because thats supposed to be inform the character, but to be fair both male and female characters tend to be drawn quite idealised unless there is a reason not to
