I think it has something to do with the eyebrows and teeth. That expression says, "Oh, you make me so mad!" which in some bizarre way I associate with little kids on a playground.

What helps me not see it is her hair. But even when I cover that with my hand, making her look bald, I still don't see it. I just see a younger, no-wrinkled Cassandra Nova.
